As a Taylor Diversion Programs (TDP) Life Coach, you will assist in the development, coordination, implementation and review of individualized service plans (ISP'S) for youth within our residential, independent living program. Additionally, you will provide and coordinate mental health services including individual, group and family counseling. Your role includes being responsible for the safety, security and proper care of the youth during the assigned shift including orientation, program planning, modeling, youth supervision and general care. All of our team members are expected to demonstrate behaviors and practices of cooperation; additionally, they must be willing to transport youth in their personal vehicles and abide by state and federal transportation laws.

Duties and Responsibilities:

Residential Youth Care:

· Review youth referral information

· Coordinate assessment process

· Consult with youth and family

· Coordinate treatment activities and services

· Follow up on youth activities for appropriateness and progress

· Make recommendations for alternative initiatives as needed

· Provide individual and group counseling

· Demonstrate leadership through role modeling

· Provide crisis intervention

· Consult with the youth, family and TDP treatment team members, mental health service providers, government and community officials (courts, court of juvenile probation, police and education authorities).

· Coordinate discharge presentation

· Coordinate with the Clinical Coordinator in the development of an individualized post facility transition plan or plans and make appropriate referrals to outside agencies for post transition plans.

Case Management:

· Maintain, distribute and file documentation as required on treatment activities and youth progress.

· Secure proper documentation for youth files meeting Federal and state requirements and organizational standards.

· Facilitate communications on youth status with essential personnel

· Manage clinical schedule

· Prepare reports as required

Qualifications: To perform this job successfully, an individual must be able to perform each duty/ responsibility satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or abilities required:

SKILLS AND ABILITIES:

· Skilled in assessing and reporting changes in youth behavior and/or affect.

· Demonstrate ability to effectively interact with individuals and groups internal and external to the organization demonstrating a positive attitude and competence level.

· Demonstrate ability to analyze an issue/problem, make recommendations for an effective and efficient resolution while maintaining the aptitude to seek alternate advice.

· Knowledge of PA 3800 Regulations as they relate to youth documentation.

· Knowledge of HIPPA privacy regulations

EDUCATION AND EXPERIENCE:

· Bachelor's Degree or Higher

· Three (3) years of experience in a related field - preferred;

REQUIRED CERTIFICATES/CLEARANCES:

· First Aid/CPR training

· PA Child Abuse and Criminal Background Clearance

· FBI Clearance (DHS)

· Safe Crisis Management (SCM) training (Offered during Orientaation)

· Valid Driver’s License
