Job Description

CenterWell Home Health is seeking a compassionate and dedicated Home Health Social Worker, PRN, to support patients and families receiving care in the comfort of their homes. In this role, you will provide clinical social work services that promote patient autonomy, safety, and quality of life, aligning with CenterWell’s mission to deliver personalized, in-home care. You will conduct comprehensive psychosocial assessments to identify emotional, social, financial, and environmental needs, collaborating closely with nurses, therapists, physicians, and other members of the healthcare team to develop individualized care plans. Working directly in patients’ homes, you will provide counseling and emotional support to patients and caregivers coping with illness, disability, and end-of-life issues. You will connect patients and families with appropriate community resources and support services, including financial assistance programs, transportation, long-term care planning, mental health services, and caregiver support. You will advocate for patient rights and preferences, ensuring that care plans reflect their goals, values, and cultural background. As a PRN team member, you will enjoy a flexible, as-needed schedule while contributing to a culture built on compassion, integrity, and excellence. You will document all visits, assessments, and interventions accurately and timely in the electronic health record, in accordance with regulatory standards and CenterWell policies. You will participate in interdisciplinary case conferences, provide input on patient progress, and help identify barriers to care, working collaboratively to resolve them. This position offers the opportunity to practice independently in the field while remaining fully supported by a collaborative healthcare team dedicated to high-quality patient outcomes. You will help educate patients and families about available services, advance care planning, and strategies to manage social determinants that impact health, such as housing instability, food insecurity, and caregiver burnout. You will play a key role in reducing hospital readmissions and supporting safe transitions of care from hospital or facility to home.
